What is Support Coordination?
Support Coordination is a crucial service and aims to assist NDIS participant in effectively utilising their funding and achieving their goals.
As part of Support Coordination function, we collaborate with participants to develop a personalised support plan that aligns with their objectives. We act as guides and advocates in helping participants navigate the complexities involved and connect them with appropriate services and supports. We closely work with participants, their families, and support networks to understand their unique needs, goals, and aspiration.
Our responsibilities as support coordinator includes the following
- Understand the participant’s NDIS plan and assist them in implementing the same by coordinating and organising necessary support and services, ensuring access to relevant resources.
- Help identify and connect with suitable disability service providers who meet the needs of the participant.
- Support participant in deciding the budget, negotiating prices and service agreements.
- Coordinate with different service providers to ensure seamless and integrated support experience.
- Assist in addressing challenges or obstacles faced while implementing support plans and also assist in NDIS plan review process.
- Regularly review and report progress towards defined goals, ensuring supports remain effective and align with evolving needs.
- Support participants in developing skills and knowledge to exercise choice and control over their supports and services.
